CarePartners is seeking a compassionate and detail-oriented Registered Nurse Case Manager for our Inpatient Rehab setting within the Healthcare & Medical Services department. In this role, you will coordinate and manage the care of patients admitted to our inpatient rehabilitation programs, ensuring safe, timely, and effective transitions across levels of care. Working closely with physicians, therapists, social workers, and other members of the interdisciplinary team, you will develop, implement, and regularly update individualized care plans that promote optimal recovery, functional independence, and positive patient outcomes. You will perform comprehensive nursing and functional assessments, identify patient needs and goals, and collaborate with patients and their families to support education, engagement, and informed decision-making. The RN Case Manager will monitor progress toward rehabilitation goals, evaluate responses to treatment, and advocate for appropriate services, including therapies, specialty consultations, and community-based resources. You will facilitate utilization management by ensuring that services are medically necessary, evidence-based, and aligned with payer requirements, while minimizing avoidable delays in care. As part of CarePartners’ commitment to community health improvement, you will embody collaboration, compassion, and patient-centered care in every interaction. You will communicate regularly with external providers, health plans, and post-acute facilities to support coordinated care, seamless discharges, and reduction of readmissions. The RN Case Manager will document all assessments, plans, and interventions accurately in the electronic health record and participate in quality improvement initiatives focused on safety, outcomes, and patient experience. This position is ideal for a skilled inpatient rehab nurse with strong clinical judgment, excellent communication skills, and a passion for helping patients regain function and independence. You should be comfortable working in a fast-paced, interdisciplinary environment, managing multiple cases simultaneously, and balancing direct patient interaction with care coordination and documentation responsibilities.
